ART MAUI 2010 ENTRY FORM  ~ Instructions to Entrant

Online registration is closed, but you may fill out and print this form and bring it with to the gallery to register for the Art Maui 2010 show. Or you may use a similar form on the printed Prospectus (PDF file). This form provides us with information about you and your entries.

If you use the printed form, the information you enter into it will be taken from that form on Receiving Day and entered into a computer for printing of labels to be attached to your work and to print a receipt which you will receive.

Your name, address, at least one phone number, and information about your first piece are required information. Your name, together with the information about your entries, will be used in Art Maui materials should your work be selected by the juror for exhibition. Your telephone number is required in case we need to contact you for any reason about your entry. Your email address will be used to send you next year's prospectus. Your mailing address is kept on file in the event our e-mailings do not reach you. Although not required, we would also like you to provide your cell phone number if you have one. The information you provide will be used only by Art Maui as described above and will not be given to any other person or organization without your consent.

Upon entering your work, you will verify and sign a printed form filled in with the information provided here or at the time of registration. Your signature is your certification that you understand the conditions for entering art work into Art Maui 2010 and that you are meeting those conditions.

The certification below will be on the printed form you sign upon entry.

I certify that I am at least 18 years of age, a resident of Maui County and that all of my Art Maui entries are original, have been created independently, and were created within the last two years without supervision. I further certify that my works have not been exhibited in any venue in Maui County, and I am not entering a piece from an edition if any number within that edition has been shown.

I understand and agree that Art Maui and/or the Maui Arts and Cultural Center will not be responsible for any loss or damage to the works hereby submitted for consideration. I will indemnify, defend and hold harmless Art Maui and the Maui Arts and Cultural Center for any loss or damage to my entry(s).

My entry into the exhibition implies my acceptance of all conditions stated in the Prospectus.
